Problem solved. I replaced the fan clutch (wasn't getting roar at start-up, even though new) and the water pump (brand new, out of box Aisin). Everything tightened to correct torque with Loctite red/blue sticks. Runs like a top. Smoother than the wife's Mitsubishi 4-banger. Drove it over 500 miles this weekend and no problems. Head gasket success, I reckon.

It was the water pump.

Brand new Aisin and started knocking within 40 miles of use. I reused the pulley and the fan. The fan was brand new Toyota. I stripped and repainted the pulley, so I know it wasn't wonky. I can't image the fan clutch could knock, but I replaced it too. The belts were brand new Toyota. I also confirmed torque on the crank bolt.
