yes, but only the Bandirante(sp) and only for a few years(late 90's, early 2000?) for the 4 door, there was a 2 door crew cab made for much longer.

TLC brought a few(3-4) of them into the states, changed out the drivetrains and some other bits and sold 2 of them, they went for $70-80K I recall, TLC kept one I think.

But Toyota(as in from Japan) never offered a crew cab 45 of any sort. There are some Aussie and German conversions(this might have been close to factory made, not sure) that were crew cabs or 4 doors.
 
Someone in Australia made conversions for the utility companies.

bottom right white 45 now belongs to a gal freinds dad, she tripped me out when she showed me the pics, as I had seen it before.
