has any of you ever had a ECU go out on you? Also what could have been the cause?

Check with NocalFJ60. He helped me troubleshoot mine (FJ60). I don't know if he dives into the 62 computers, but he knows his way around the 60's.

I never had one fail but i imagine water or a ground issue could cause failure.
