Looking to get some wheels for the 40. I know 3.5" is the magic number but won't 4.5" work? Anyone know?
 
No, too deep. If it is a 15" rim it will hit the tie rod ends. Sounds too deep for a 16" rim, too.
 
I sold a set of 16x8 4.5" BS wheels with 33" swampers on them. He bolted them up to his drum brake cruiser without a problem. YMMV.
 
I have 16" wheels with 4.5" backspacing and used Marlin Crawlers 2" wheel spacers. So far so good.

Even if it fits, I don't know if I would want my truck to be another two inches skinnier.

Also, My 33x12.50's on 8 inch rims will slightly rub the inside of the wheel well in the rear under hard stuff. Another inch in and it would be worse.
