OK so it turns out that surging is a symptom of the torque converter lockup slipping, which is due to old trans fluid. Replaced and the issue appears to have gone away!
So between writing this and now, the MIL has cleared on its own. I've cleared the code memory and we'll see what happens. Surge still present after an ECU reset unfortunately.

Found it. Odd it isn't listed in the parts diagram I have in my FSM. Replaced both on the same bank. So far the idle has straightened out nicely and CEL has held off. We'll see!
OK so I can't figure out where the O2 sensor is. The part lists include "UPSTREAM LH/RH" and "DOWNSTREAM LH/RH". Code says Bank 1 (which is LH side), sensor 1. The only parts location diagrams I see show "Sensor 2" for Bank 1 and Bank 2. Where are "Sensor 1" located for each bank???
Next time it triggers I will read the code in Techstream. It is possible my reader can't pull it (ScanGauge II). I pulled the 2 main vacuum leads at the front of the intake (I think these are the two main vacuum sources?) and plugged them one at a time. Neither had an effect on the idle surge.

Does anyone know the part number or good source for key blanks? My internals are fine but I jammed the key in the door upon closing and bent the heck out of the blade. Looks like the button face and blade are one part that can be replaced. Thoughts?

I wonder if the issue could be the upper engine ground being completely disconnected? I guess we'll never know! Guess I'll try running the truck with that reconnected for a while lol
However, the odd thing is that the truck was running with that disconnected. Obviously there are several grounds...
