   If it were a vacuum or air line of any kind it would have to be from the smog pump, because the noise stopped with the belt removed.

   3F-E Help

   Yes it happens every time it starts and runs and doesn't stop until the engine is off, also the pitch of the sound seems to be based on RPM. The engine still runs perfectly fine but the noise is nearly deafening from outside the truck.
