I picked this truck up recently and it has one major rust area I need to address before it gets nuts. My first option is to take it to a body shop but I feel they will just bondo it and it will come back in a year. I was also considering grinding it down and then using black Rust Bullet on it. It seems the metal is still solid so that is a plus. Any advice is welcome!

I’d clean it up with a wire wheel, skim some filler on there to smooth it out, prime, and paint. I’d also get underneath to make sure that is clean as well. Wire wheel, primer, and paint there if needed.

I'd cut it out and weld in a rust free piece.
The wire wheel will polish the rusty metal to make you think you removed it.

As PAToyota said, can you see behind the panel with a light and a mirror or a scope? If it's rusty from behind, it's all gotta go.

Maybe, you could sand it down to remove most of the rust, hit what remains with phosphoric acid (rust converter). I don't know what paints and/or fillers are compatible with the acid.

There is no easy correct way..........
Cut it out, weld in new metal, prepare and prime, paint.

Anything less, and you'll be rewarded with seeing the rust come back.
