I had the 2LTE engine in a Hilux Surf many years ago with most of the issues you described.
Change all the glow plugs resolved my slow starting. Replaced the hose to the fuel filter as it was allowing air to bleed in.

should I use the same style of steel gasket or something different? Do they make thicker gaskets for this application?
Thicker gaskets are available. Search for multiple layer or composite, you should find something that fits. Alternatively, sandwich a couple of your existing gaskets back to back?
Sounds very similar to when my O2 sensor started failing.
Occasional stumbling on acceleration with a very limited rev range (around 1600rpm). This stumbling got more and more frequent but not every time I drove.

I made a block off plate for where the EGR used to sit
Then I modified an old Volkswagen drain plug…..
And used the plug to block off the pipe….
I figured that as no gas is flowing the transfer of heat will be much lower 🤔
It has passed emissions testing after this mod.
I’ve looked, and looked again…… I can’t see the MAF anywhere in the IC piping so I guess you’re going MAP and controlling it through the Haltech?
If so, where are you fitting the pressure sensor?
And what about intake temps?

Mine is just your bog standard cheapo set up. Had to replace the supplied hoses with a quality brand as the vacuum was collapsing them.
Not much oil out yet but the truck has only just passed 154,000 miles so it’s barely run in 😎

Since your post, I’ve spent the last 3 days researching fitting this plenum with a turbo to a 1FZ in an LC80 (and also intercooler).
