Do you mean the engine is running rough and shaking the vehicle?
If so it may be starting on only 2-3 cyl because a glow plug is not working

Smoke on startup? What happens when you advance the throttle slightly?

Mine does something like that when it's cold. I've been guessing that it's a bad or loose engine or tranny mount, and when it's cold it jerks around a bit more and the exhaust is smacking something.

several times over the last few months, it's almost bothered me enough to crawl underneath and have a look.....

I've also had it when one or two glow plugs are out, and then it does run rough as hell and smoke for a bit at start up.

cheers,
ryan

any visible fuel leaks?

could be that when it is shut down overnight some air gets in.
