CGS form-where to forward

Eery year the TLCA requires each chapter to forward a Chapter in Good Standing form, complete with TLCA ID#s for each active member in both clubs. This form is now overdue. I could not find one in my database {i've lost 2 computers in the last two years] so I had Jennifer send me a new one.

I need an e-mail address to forward this document to. This WAS the reps job, but in the absence of one, I think maybe this job would fall to the secretary. Monica?
