It does kind of have that Iceland tiny lift/huge tire/massive fender flare look.

yeah! haha jeezum, where did you find that pic? i know its on your site, i've been there all night since i found this post.

my idea though, was to reverse the extra cab, and cut off the back window sections of both cabs, removing as much as possible without screwing up the door frame. then i'd have four doors, teh rears would be suicide for easier access. not sure how much of the cab would hang over where the rear tirewell should be. i'd probably end up cutting it out and installing flares or something.
